Submission of a pickup request does not constitute a binding service agreement. All pickup requests are subject to review, availability, and acceptance by MedRegion. Upon acceptance, services will be performed in accordance with MedRegion’s applicable service terms and operational policies.

6. Packaging and Compliance Representation

- All materials tendered for transport are properly packaged, sealed, and labeled in accordance with applicable federal, state, and local regulations.
- Any regulated materials, including biological or medical specimens, have been packaged in accordance with applicable transportation and safety standards.
- Accurate pickup and delivery information has been provided.
- The materials are lawful for courier transport.
MedRegion shall not be responsible for damages, delays, or regulatory violations resulting from improper packaging, inaccurate instructions, or undisclosed handling requirements.
7. Service Confirmation
Submission of this form constitutes a request for courier services only. Services are not confirmed until MedRegion dispatch personnel review the request and confirm acceptance.
Once confirmed, services will be performed subject to MedRegion’s applicable Terms and Conditions, which govern liability limitations, payment obligations, and service procedures.
8. Acknowledgment
By submitting this request, the requesting party acknowledges and agrees that:
- The information provided is accurate to the best of their knowledge.
- Courier services remain subject to acceptance by MedRegion.
- All services are governed by MedRegion’s Terms and Conditions.
